To identify a learning gap, triangulate what the learner produced, what they say they were trying to do, how they respond to a targeted prompt and what they can repeat independently. One wrong answer is evidence of a wrong answer; it is not yet a secure explanation of the cause.
Start with a bounded question
Define the current job before assessing it:
- “solve a linear equation with the unknown on both sides”;
- “infer an attitude and support it from an unseen extract”;
- “explain one causal link in this history question”;
- “apply this biology concept to an unfamiliar context”.
Check the relevant specification or curriculum context. Do not use a task from the wrong awarding organisation, tier or stage as if it proves a gap.
Use four sources of evidence

None is sufficient alone. A work sample may have been rushed; a verbal explanation may exceed written execution; a correct answer may depend on a prompt the learner cannot recreate.
Find the first consequential break
Work backwards from the visible error and ask:
- Did the learner interpret the task and command word?
- Is the necessary knowledge retrievable?
- Can they choose an appropriate method or structure?
- Can they execute each step accurately?
- Can they check, explain or adapt the result?
Prioritise the earliest break that explains later errors. Correcting every downstream symptom at once creates noise.
Test a hypothesis with the least help
Suppose a learner cannot complete a ratio problem. Do not immediately reteach the whole topic. Try a sequence:
- ask them to represent the known quantities;
- check the relevant multiplication/division fact;
- offer a diagram or worked comparison;
- model one decision, not the full solution;
- give a new problem with the same underlying relationship.
If a small prompt restores the process but disappears on the fresh task, the knowledge may be fragile or cue-dependent. If the learner succeeds independently, move on rather than continuing to scaffold.
Record evidence precisely
Use observable language:
On two unfamiliar examples, Sam identified the relevant values but divided in the opposite order. After comparing the units, Sam corrected one example and independently solved a fresh one. Recheck after spacing.
Avoid unsupported diagnoses about dyslexia, attention, anxiety or motivation. Tutors can notice patterns and share specific evidence through the agreed route, but assessment and pastoral decisions may belong to school staff or qualified professionals.
Turn the gap into a reviewable target
Write a target with:
- the exact task;
- current evidence;
- the next teaching/practice action;
- the independent evidence that would count;
- a review point.

Generate competing hypotheses
For one visible error, list at least two plausible explanations:

Do not teach the first explanation as fact until a small check supports it.
Work through a think-aloud without leading
Prompt:
Read the task and tell me what you notice, what you think it asks and what decision you would make first.
Follow with neutral questions:
- “What makes you say that?”
- “Which information matters?”
- “What alternatives did you consider?”
- “Where did you become uncertain?”
Avoid embedding the answer: “Don't you need to divide by the original value?” turns the check into prompted practice.
Separate curriculum gaps and access barriers
Evidence may point to:
- content not yet taught;
- forgotten prerequisite;
- misunderstood concept;
- task-language difficulty;
- working-memory or organisation burden;
- inaccessible format or technology;
- confidence/avoidance around a task;
- broader concern outside tutor remit.
A tutor can adapt teaching and report observations. Do not diagnose SEND/ALN, health or motivation. Use the agreed school/parent/professional route.
Use a minimum sufficient gap map
Record only gaps that change the next teaching decision:

Long diagnostic spreadsheets can create the appearance of precision while delaying useful teaching.
Revisit the hypothesis
Change it when:
- the learner succeeds in a new context;
- a school work sample contradicts it;
- the same error appears for a different reason;
- support does not fade after appropriate teaching;
- current specification or teaching sequence changes the task.
Good diagnostic teaching remains provisional and responsive.
